Why our targets sit higher than the lab's
A lab reference range describes the middle 95 percent of the people it tested. It does not describe how those people felt. Many men with fatigue, low drive, and slow recovery sit inside it and get told nothing is wrong.
But total testosterone alone misses a lot. Only the free fraction reaches tissue, and SHBG decides how much of your total stays locked up. That is why this tool calculates free testosterone with the Vermeulen equation, the method most published calculators use, and why LH and FSH sit at the top of the diagram. They tell us where the slowdown starts.
Targets are a starting point for a conversation. Symptoms, safety labs, and your goals, including fertility, decide the plan.

Questions
What is calculated free testosterone?
Most testosterone in your blood is bound to SHBG or albumin. The small unbound share is free testosterone, the part your tissues can use. This tool estimates it from your total testosterone, SHBG, and albumin using the Vermeulen equation. If you leave albumin blank, it uses 4.3 g/dL. If your lab measured free testosterone directly, enter it and the tool will use your measured value instead.
Are my results stored anywhere?
No. The math runs in your browser. Nothing is transmitted, logged, or saved, and refreshing the page erases it.
Can I use this if I'm already on testosterone?
Yes, and you should tell the tool. Testosterone therapy lowers LH and FSH on purpose, so low values there are expected rather than alarming. Timing also matters. A level drawn the day after an injection reads very differently from one drawn right before the next, so note when you last dosed and tell your provider.
Why does my lab flag a value this tool calls fine, or the reverse?
Your lab compares you to a population average. We compare you to the range where our patients tend to feel and function best, while still watching safety markers like hematocrit and PSA. Both views are useful. Neither replaces a clinician reading them together.
Does this tool diagnose low testosterone?
No. A diagnosis takes symptoms, at least two morning measurements, a physical exam, and a look at causes like sleep apnea, medications, and thyroid function. Use this to understand your numbers and to ask better questions.
