Tirzepatide is a dual GIP and GLP-1 receptor agonist. It slows gastric emptying, signals fullness earlier, and reduces the constant background pull toward food that patients describe as the hardest part of eating less. That last effect is the one patients tend to notice first.
It is a prescription medication, not a supplement, and it works best alongside adequate protein, resistance training, and sleep. We will talk about all three, because losing weight without protecting muscle is a poor trade.

The Honest Comparison
This is the question almost everyone asks and almost nobody answers straight. Both are effective. They are not identical.
Tirzepatide acts on the GLP-1 receptor. Semaglutide acts on GLP-1 and GIP. In head-to-head trial data, the dual mechanism has produced greater average weight loss.
Trial averages describe populations. Plenty of people do extremely well on semaglutide and never need to switch at all. Response is individual, which is why we titrate rather than assume.
Tirzepatide costs more, at $299 compounded and $299 brand, against $199 compounded and $349 brand for semaglutide. Compounded, that is about $1,200 a year in difference. Brand-name, tirzepatide is actually the cheaper of the two.
Nausea, reflux, and constipation are common with both, usually worst during dose increases. Some people tolerate one noticeably better than the other, and that is a legitimate reason to change.
Our position. we start patients on semaglutide unless there is a reason not to, then move to tirzepatide if response stalls or tolerance is poor. Tirzepatide is a strong first choice when someone has already failed semaglutide, has a lot to lose, or has significant insulin resistance. Starting on the more expensive option to look aggressive is not clinical care.

Insurance may reduce it further. We do not mark up brand-name medication and we do not take a cut of it. Our fee above covers the clinical care around it, and nothing else. You are paying for evaluation, monitoring, dose decisions and refill authorization, not for a prescription. What we prescribe is always a clinical decision. Labs are not included in brand-name management and are priced separately.

Zepbound self-pay pricing through LillyDirect requires refills be completed within 45 days of your previous supply to maintain the lowest price tier.
To protect your pricing, submit your refill request at least 7 business days before day 38 of your current supply. Monthly management patients are contacted before that window opens, so in practice you should not have to track it yourself. That gives us time to review and send it, and the pharmacy time to process and ship before your window closes. Requests received later may result in a supply gap or loss of self-pay eligibility.
