Why Thyroid Gets Missed

TSH Alone Does Not Tell the Full Story.

The standard approach to thyroid testing in most primary care settings is to run a TSH and, if it falls within the reference range, declare the thyroid normal. For many patients, this misses what is actually going on.

TSH is a pituitary signal, not a direct measurement of thyroid hormone activity. Free T3 is the active thyroid hormone that cells actually use. A normal free T4 with a low free T3 points to a conversion problem rather than a thyroid that is failing. Antibodies can indicate autoimmune thyroid disease, or Hashimoto's, that TSH will not catch until significant damage has occurred.

Treatment Options

Thyroid Treatment at Navara Health.

Treatment approach depends on your specific lab findings, symptoms, and history. Not every thyroid issue requires medication. Your provider will make individualized recommendations based on your full picture.

Levothyroxine (T4)

Standard thyroid replacement therapy. Appropriate for many patients with hypothyroidism. Dosed based on weight, labs, and symptoms rather than a fixed formula.

Liothyronine (T3)

Direct T3 replacement for patients who do not convert T4 to T3 adequately. Often considered when Free T3 is low or symptoms persist on T4 alone.

Compounded T3 and T4 Combination

Customized combination of T3 and T4 from a licensed 503A compounding pharmacy. Used when standard medications are not providing adequate symptom relief.

Desiccated Thyroid (NDT)

Natural desiccated thyroid containing both T3 and T4. Preferred by some patients. Discussed at consultation based on your clinical picture and preference.

Treatment eligibility is determined at your consultation and is not guaranteed in advance. Thyroid medication is prescribed at provider discretion following review of your full panel, symptoms, and health history. Compounded T3 and T4 preparations are prepared by licensed 503A compounding pharmacies and are not FDA-approved in the same manner as commercially manufactured drugs. Natural desiccated thyroid is not FDA-approved for the treatment of hypothyroidism and is prescribed off-label. Risks, benefits, and alternatives are reviewed with you before anything is prescribed.

Some patients convert T4 poorly due to gut issues, stress, selenium deficiency, or inflammation. We read free T3 against free T4 rather than either one alone, and we check ferritin, because low iron impairs the conversion directly.

Hashimoto's is the most common cause of hypothyroidism and is often undiagnosed for years. TPO and thyroglobulin antibodies are included in our standard panel.

After that, ongoing management is $100 per month if you are already on a Navara Health hormone, testosterone, or weight loss membership, or $149 per month if thyroid is the only thing you see us for.

You do not need to be a hormone patient to be treated for thyroid. If you already are one, adding thyroid is the cheaper path.

My TSH was normal but I still feel bad. Can you help? ▾
Yes. This is one of the most common reasons patients come to Navara Health for thyroid care. A normal TSH does not rule out thyroid dysfunction. Free T3 and thyroid antibodies can both be abnormal while TSH appears normal. We run the full panel and treat based on your complete picture and how you actually feel.

What is Hashimoto's thyroiditis? ▾
Hashimoto's is an autoimmune condition where the immune system attacks the thyroid gland. It is the most common cause of hypothyroidism and is often undiagnosed for years because TSH can remain normal until significant thyroid damage has occurred. TPO and thyroglobulin antibodies are the key markers. We include both in our standard thyroid panel.
What is the difference between T3 and T4? ▾
T4 is the storage form of thyroid hormone the thyroid produces in large quantities. It must be converted to T3 to become active. T3 is the form cells actually use. Some people do not convert T4 to T3 efficiently, which means they can have normal T4 and TSH but inadequate T3 activity at the cellular level. This is why Free T3 is an important part of a complete evaluation.
Is thyroid treatment available via virtual care? ▾
Yes. Thyroid consultations and ongoing management are available by virtual care in 11 states through Jessica Boggs, APRN: Texas, Arizona, Colorado, Connecticut, Florida, Iowa, New Mexico, Oklahoma, Vermont, Virginia, and Washington. Labs are ordered through Quest Diagnostics and drawn near you. Prescriptions are filled by a licensed pharmacy.
Do I have to be on hormone therapy to receive thyroid treatment? ▾
No. Thyroid optimization is open to patients who are not on any other membership. Everyone starts at $199 including labs and the first month of medication. Ongoing care is then $149 per month for thyroid-only patients, or $100 per month if you are already on a Navara Health hormone, testosterone, or weight loss membership. If you are already a patient, mention thyroid at your next visit and your provider will fold it into your existing plan.

Before booking, please review our Patient Policies and Financial Agreement and our Virtual Care and Communication Policies. New patients also complete the General Intake and Consent Form before their appointment. By scheduling, you acknowledge that you have reviewed and agree to these policies.

Laboratory Work

Your $199 Thyroid Start includes the full thyroid panel, ordered and paid for by us. After that, a dose change calls for a recheck at six to eight weeks. When that happens you have three ways to handle it.

Included in the program

Your full baseline thyroid panel: TSH, free T4, free T3, TPO antibodies, thyroglobulin antibodies, and ferritin, plus a CMP and CBC. Ordered, paid for, and scheduled by us. Nothing is billed to you for it.

Use your insurance

We send the order to Quest or LabCorp and they bill your plan directly.

We cannot guarantee coverage. Your plan, your deductible, and how your insurer classifies the test decide what you owe. Insurers frequently deny extended thyroid testing as not medically necessary. Call the number on your card or contact Quest before your draw to confirm.

Bring labs from your PCP

If your primary care provider orders similar testing and it is covered under your plan, bring us the results.

As long as they are current, include the markers we need, and come from an accredited laboratory, we will use them. This is often the least expensive route, and we will tell you exactly what to ask your PCP to order.

Pay cash

We build a bundle of only the markers your situation calls for rather than a full panel.

You get the price before the order is placed, so there are no surprises. A single follow-up marker is often under $30.

Rechecks after a dose change are not optional. Thyroid hormone takes roughly six weeks to reach a steady state, so a dose change without a follow-up panel is a guess. Where a test is required for safe prescribing, we will tell you it is required rather than optional, and treatment continues once it is completed.
