The standard approach to thyroid testing in most primary care settings is to run a TSH and, if it falls within the reference range, declare the thyroid normal. For many patients, this misses what is actually going on.

TSH is a pituitary signal, not a direct measurement of thyroid hormone activity. Free T3 is the active thyroid hormone that cells actually use. Reverse T3 can block T3 even when levels appear adequate. Antibodies can indicate autoimmune thyroid disease (Hashimoto's) that TSH will not catch until significant damage has occurred.

Yes. This is one of the most common reasons patients come to Navara Health for thyroid care. A normal TSH does not rule out thyroid dysfunction. Free T3, Reverse T3, and antibodies can all be abnormal while TSH appears normal. We run the full panel and treat based on your complete picture and how you actually feel.
What is Hashimoto's thyroiditis?
Hashimoto's is an autoimmune condition where the immune system attacks the thyroid gland. It is the most common cause of hypothyroidism and is often undiagnosed for years because TSH can remain normal until significant thyroid damage has occurred. TPO and thyroglobulin antibodies are the key markers. We include both in our standard thyroid panel.
What is the difference between T3 and T4?
T4 is the storage form of thyroid hormone the thyroid produces in large quantities. It must be converted to T3 to become active. T3 is the form cells actually use. Some people do not convert T4 to T3 efficiently, which means they can have normal T4 and TSH but inadequate T3 activity at the cellular level. This is why Free T3 is an important part of a complete evaluation.
