Telemedicine Consent

By receiving telehealth services from Navara Health, PLLC, you acknowledge and agree to the following regarding telemedicine care provided by Jessica Boggs, MSN, APRN, FNP-C, ENP-C and clinical staff working under her delegation, practising under the medical direction of Simal Patel, MD.
Navara Health is a medical practice, not a technology platform. Your provider is directly responsible for your care and for the clinical decisions made about it.
What telehealth is
Telehealth is the delivery of healthcare using electronic communication and information technology, allowing a provider and patient in different locations to share medical information for the purpose of clinical care, diagnosis, consultation, treatment, and education.
Telehealth services at Navara Health may include:
- Secure online questionnaires reviewed by your provider, sometimes called asynchronous or store-and-forward care
- Telephone consultations
- Real-time HIPAA-compliant video visits
- Remote monitoring, laboratory review, and follow-up care
- Electronic prescribing
- Medical advice and health education
Your provider decides which format is appropriate based on what you are being seen for, your history, what your state requires, and whether anything needs to be discussed or examined directly.
Expected benefits
- Access. Receive care without travelling to a clinic
- Convenience. No travel time and no waiting room
- Continuity. Easier follow-up and ongoing monitoring with the same provider
- Efficiency. Streamlined evaluation and management of ongoing conditions
Risks and limitations
- Technology failures. Equipment, internet, or software failures may delay evaluation or treatment
- Quality limitations. Video, image, or audio quality may be inadequate for proper clinical decision-making, which may require rescheduling or an in-person visit
- Incomplete information. Your provider will not have your complete medical history unless you provide it or authorise a records transfer, which may contribute to adverse drug interactions, allergic reactions, or other errors
- Security risks. Despite security protocols, there is a rare possibility of a privacy breach or unauthorized access to your health information
- Examination limitations. The inability to perform a physical examination may affect the ability to diagnose certain conditions
- Emergencies. Telehealth is not appropriate for emergency medical conditions
Your rights
- Know your provider. Review the credentials and qualifications of the provider delivering your care
- Privacy. Expect that information shared during telehealth is kept confidential and protected under HIPAA and Texas law
- Ask questions. Ask about telehealth, how it works, and what to expect, at any point
- Withdraw consent. Withdraw consent to telehealth at any time without affecting your right to future care or losing any benefit you would otherwise be entitled to
- Alternatives. Be informed of alternative options, including in-person care
- Your records. Request copies of your medical records in accordance with applicable law
In-person care is a genuine alternative here. Navara Health maintains a clinic in Dallas, Texas. If you would prefer to be seen in person for a service we provide in clinic, tell us and we will schedule it. You are not required to accept telehealth.
Your responsibilities
- Accurate information. Provide complete and accurate medical history, current medications, allergies, and health information
- Honest communication. Answer all questions truthfully and completely
- Compliance. Follow the treatment plan agreed with your provider, and tell us if you stop or change something
- Private environment. Be in a private, quiet location during video visits
- Technology. Have the necessary device and internet connection to participate
- Know when to seek emergency care rather than using telehealth
Who provides your care
All clinical services are provided directly by Navara Health. Jessica Boggs, MSN, APRN, FNP-C, ENP-C is licensed as an Advanced Practice Registered Nurse and holds active licensure in each state where telehealth is offered. Certain in-clinic aesthetic services are performed by Rocio Gonzalez, RN under APRN delegation. All care is delivered under the medical direction of Simal Patel, MD.
Navara Health is responsible for the quality and appropriateness of the care provided to you. You will be told who is treating you before your visit.
Recording and documentation
All clinical information exchanged during your visit is documented in your electronic medical record.
Clinical or administrative staff may be present during your consultation for scheduling, billing, or clinical support purposes. You will be informed if anyone else is present, and you may ask that they leave.
Recording and AI-assisted documentation
Our electronic medical record system is HIPAA-compliant and has the capability to record consultations, and we use AI-assisted clinical documentation, sometimes called an AI scribe, to help produce your visit note.
What that means in practice:
- Audio from your visit may be captured and processed to draft your clinical note.
- Your provider reviews and edits every note before it is finalised. The AI drafts, a human decides. Nothing enters your record unreviewed.
- Recording and AI documentation happen inside a HIPAA-compliant system under a business associate agreement. Your information is not used to train public AI models.
- You will be told before any recording begins, and you may decline. Declining does not affect your care or your access to treatment. Your provider will simply document the visit manually.
- You may ask at any point during a visit that recording stop.
- Recordings and notes are part of your medical record and are retained and protected in the same way as the rest of it.
Prescriptions
Medications are prescribed only when clinically appropriate based on your provider's professional judgment. There is no guarantee that a prescription will be written, and treatment eligibility is determined at your evaluation.
Controlled substances
Navara Health prescribes testosterone, which is a Schedule III controlled substance. Under Texas law and DEA requirements, testosterone therapy requires an in-person evaluation before it can be prescribed, and is available to Texas patients only.
It cannot be started or managed by telehealth alone, and it is not available to patients in our other licensed states. If your evaluation points toward testosterone therapy and you are outside Texas, we will tell you plainly and discuss alternatives, including enclomiphene, which is not a controlled substance and can be managed by telehealth in all eleven states.
Your provider may require in-person follow-up or laboratory testing before prescribing or continuing certain medications. We reserve the right to decline to prescribe where there is concern about misuse.
You always have the right to have your prescription sent to the pharmacy of your choice. Where we recommend a partner pharmacy, that is a recommendation and not a requirement.
No guaranteed results
Medicine is not an exact science and no provider can guarantee a specific result. Using telehealth does not guarantee that your condition will improve or resolve. Treatments may have side effects or complications. You may require in-person care as a follow-up to a telehealth visit.
Several treatments we offer are prescribed off-label, meaning they are not FDA-approved for the specific use prescribed. Off-label prescribing is legal and common when supported by clinical judgment and informed consent, and it means the evidence base is smaller and individual response varies more. Risks, benefits, and realistic expectations are reviewed with you before anything is prescribed.
Limitation of liability
You agree to hold Navara Health harmless for the following, which are outside our reasonable control:
- Technical failures of equipment, software, or internet connectivity
- Delays in evaluation or treatment caused by technology problems
- Breaches of security protocols despite reasonable safeguards
This section does not limit our professional responsibility for the clinical care we provide. Clinical judgment and treatment decisions are the responsibility of your Navara Health provider and are governed by the applicable standard of care and by Texas law.
Emergencies
Telehealth is not appropriate for emergency medical conditions
If you are experiencing a medical emergency:
- Call 911 immediately, or
- Go to the nearest emergency room, or
- Call or text 988, the Suicide and Crisis Lifeline, for a mental health emergency
Examples of emergencies include chest pain, difficulty breathing, severe bleeding or trauma, loss of consciousness, severe allergic reaction, stroke symptoms such as facial drooping, arm weakness, or speech difficulty, and thoughts of harming yourself or someone else.
For urgent but non-emergency concerns outside our hours, contact your primary care provider or visit an urgent care centre. We do not monitor messages continuously.
Where we can treat you
Telehealth is available only in the states where Jessica Boggs, APRN holds active licensure: Texas, Arizona, Colorado, Connecticut, Florida, Iowa, New Mexico, Oklahoma, Vermont, Virginia, and Washington.
You must be physically located in one of these states at the time of your visit, not simply a resident of one. If you are travelling, tell us, because we may not be able to see you until you return.
If you move to a state where we are not licensed, we cannot continue to provide care and will help you transition to another provider.
Filing a complaint
If you have a concern about your care, please contact us first at (469) 653-3124 so we can address it. You also have the right to file a complaint with the appropriate licensing board.

Laboratory testing
Laboratory testing ordered as part of your telehealth care is performed by Quest Diagnostics or another accredited reference laboratory. Your requisition is sent to you securely and you complete the draw at a location near you.
Results are information, not a diagnosis. They are reviewed by your Navara Health provider and discussed with you. Where testing is billed to your insurance rather than paid at our rate, coverage cannot be guaranteed and the laboratory bills you directly.
Updates to this consent
We may update this consent to reflect changes in our practices, technology, or legal requirements. Material changes will be communicated through our website or by email. Continued use of telehealth services after such changes constitutes acceptance of the updated consent.

By proceeding with registration or a consultation with Navara Health, you acknowledge that:
- I have read and understand this Telemedicine Consent
- I understand what telehealth is and how it works
- I have had the opportunity to ask questions about telehealth
- I understand the potential benefits, risks, and limitations
- I understand that I may withdraw consent to telehealth at any time without affecting my right to care
- I am aware that in-person care at the Dallas clinic is an alternative for services provided there
- I consent to receive telehealth services from Navara Health
- I understand that Navara Health, and not any third party, is responsible for my medical care
- I understand that testosterone is a controlled substance requiring an in-person evaluation and is available to Texas patients only
- I understand that some treatments are prescribed off-label and that no specific result can be promised
- I understand that telehealth is not appropriate for emergencies and that I should call 911 or 988
- I have been informed how to request copies of my medical records
- I understand the limitations of liability described in this consent
- I am physically located in a state where Navara Health is licensed at the time of my visit
- I will provide accurate and complete health information and update it as it changes
